The quote form on this site is Gelpax’s cold-chain quote survey, hosted by GoHighLevel. It collects the details you enter there so Gelpax can respond about shipping ice packs and related sourcing. Do not include patient information, financial credentials or other sensitive personal data.

Request context and lead handling

The embedded survey receives source attribution for shippingicepacks.com, the page path where you opened the form, and any utm_* campaign parameters present in the page URL. Avoid putting personal or sensitive information in URL parameters. Submitted inquiries go into Gelpax’s shipping inquiry workflow in GoHighLevel (Shipping Ice Packs Customers pipeline).
